Before receiving care through Calibr Health, please read and understand the following information about telemedicine, your rights, and how we deliver care.

1. What is telemedicine?
Telemedicine (also called telehealth) is the delivery of healthcare services using electronic communications — including secure messaging, asynchronous health questionnaires, and video or phone consultations — in place of an in-person visit. It allows you to receive care from a licensed clinician without traveling to a physical office.
2. How Calibr delivers telemedicine
Calibr Health provides access to licensed clinicians through a combination of asynchronous and synchronous care:
- Asynchronous: You complete a secure online health questionnaire and submit lab results; your clinician reviews and responds at a later time, typically within 1–2 business days.
- Synchronous: When clinically appropriate, you may have a live video or phone consultation with your clinician.
All clinical communication takes place on a HIPAA-compliant platform operated by our clinical partner, OpenLoop Healthcare Partners.
3. Risks of telemedicine
By consenting to telehealth, you acknowledge and understand the following risks and limitations:
- Not all conditions can be evaluated remotely. Some medical conditions require an in-person physical exam, imaging, or specialist care. Your clinician may determine that telehealth is not appropriate for your situation and refer you elsewhere.
- Technology may fail. Internet outages, device failures, or platform issues may delay or interrupt care.
- You may need in-person care. If a serious or emergent condition is identified, your clinician will recommend in-person evaluation. Calibr is not a substitute for an established relationship with a primary care provider.
- Information limitations. The quality of remote care depends on the accuracy and completeness of the information you provide. Omitting medical history, medications, or symptoms may affect your treatment plan.
4. Benefits of telemedicine
- Convenience. Receive care from anywhere with an internet connection.
- Expanded access. Reach clinicians who specialize in men's hormone health without geographic limitations.
- Time efficiency. No waiting rooms, no travel — most interactions take a few minutes.
- Continuity of care. Secure messaging and quarterly follow-ups support ongoing treatment monitoring.
5. Your rights as a patient
- Withdraw consent. You may withdraw consent for telehealth services at any time without affecting your right to future care.
- Request in-person referral. You may ask your clinician for a referral to an in-person provider at any time.
- Access your records. You have the right to access your medical records and request copies. See our privacy policy for HIPAA access rights.
- Request privacy. You may request alternative or confidential communications methods (e.g., a different email address or phone number).
- Receive care without coercion. Your decision to use telehealth is voluntary. You are never obligated to continue care.
6. How your health data is handled (HIPAA)
All Protected Health Information (PHI) collected through Calibr Health is handled in accordance with HIPAA and our Privacy Policy. Health data is stored in HIPAA-compliant systems operated by OpenLoop Healthcare Partners as our Business Associate. Data is encrypted in transit and at rest. Access is restricted to authorized personnel on a need-to-know basis.
7. Identity verification
To comply with state telehealth regulations and federal prescribing requirements, Calibr verifies the identity of every patient before any clinical evaluation. Identity verification is performed through Plaid Identity Verification using a government-issued ID and a selfie liveness check. Your identity verification data is processed by Plaid in accordance with their privacy policy and is not used for any non-clinical purpose.
8. Clinician identity
You will be told the name, license number, and state of licensure of the clinician assigned to your case before any clinical encounter. You may request a different clinician at any time by emailing support@calibr.health.
9. How to reach your clinician
Once enrolled, you can message the Calibr support team through your patient portal. Support routes all clinical questions to your prescribing clinician. Non-urgent clinical responses are returned within 1 business day. Urgent or emergent issues should be directed to 911 or your nearest emergency room.
10. Acknowledgement and consent
By using the Calibr Health services — including completing the online health questionnaire, submitting lab work, accepting a prescription, or otherwise engaging with our clinical platform — you acknowledge that you have read and understood this Telehealth Informed Consent, and you consent to receiving healthcare services via telemedicine as described above.
