Terms of Service.

By using Calibr Health, you agree to these Terms of Service. Please read them carefully.

1. Acceptance of Terms

By accessing or using Calibr Health's website, services, or mobile applications (collectively, the "Services"), you ("Patient" or "User") agree to be bound by these Terms of Service ("Terms") and our Privacy Policy. If you do not agree, do not use the Services.

2. Eligibility

To use the Services, you must:

  • Be at least 18 years of age
  • Be a resident of the United States in a state where we offer services
  • Have the legal capacity to enter into a binding contract
  • Provide truthful, complete, and accurate information

The Services are intended for adult men only. If you are not eligible, you may not use the Services.

3. Description of Services

Calibr Health operates a consumer-facing telehealth platform that connects patients with licensed clinicians for the evaluation and treatment of low testosterone (TRT) and related men's health conditions. Through our Services, eligible adult patients can:

  • Complete an asynchronous health intake and medical history
  • Order at-home laboratory testing
  • Receive an asynchronous or synchronous clinical consultation with a licensed clinician
  • Receive a prescription if clinically appropriate, dispensed by a licensed pharmacy partner
  • Receive ongoing monitoring, follow-up consultations, and care-team messaging
Calibr Health is a technology platform, not a medical practice. All clinical services — including medical evaluations, diagnoses, prescription decisions, and treatment management — are performed by licensed, independent clinicians who use the OpenLoop Healthcare Partners platform. These clinicians exercise independent professional medical judgment in all clinical matters.

4. Not a Replacement for Emergency Care

If you are experiencing a medical emergency, call 911 immediately or go to the nearest emergency room. Calibr Health is not for emergency or urgent care. Do not use the Services to seek treatment for life-threatening conditions, severe pain, suicidal thoughts, or any situation requiring immediate medical attention.

5. No Guarantee of Prescription

Enrollment in Calibr Health does not guarantee that you will be prescribed testosterone or any other medication. Our clinicians evaluate each patient individually based on medical history, symptoms, and lab results, and only issue a prescription when, in their independent clinical judgment, treatment is medically appropriate. If you are not a candidate for treatment, you will be informed and may be eligible for a partial refund as described in our refund policy.

6. Telehealth Consent

By using the Services, you consent to receiving healthcare services via telehealth as described in our Telehealth Informed Consent. You acknowledge that telehealth involves the use of electronic communications to deliver care, that not all conditions are suitable for remote evaluation, and that you may always seek in-person medical care from a local provider. We encourage you to maintain a relationship with a primary care provider.

7. Subscription Terms

Calibr Health is offered as a monthly subscription at $199 per month, payable in advance. By enrolling, you authorize Calibr Health to charge your selected payment method on a recurring basis until you cancel. Subscription fees include clinician consultations, at-home labs, prescription medication (when clinically appropriate), ongoing monitoring, unlimited care-team messaging, and free monthly delivery.

All payments are processed via Stripe, Inc. Calibr Health does not bill insurance — services are cash-pay only.

8. Cancellation Policy

You may cancel your subscription at any time through your patient portal or by emailing support@calibr.health. Cancellation takes effect at the end of your current billing period — you will not be charged for the following month, and your access to clinical services ends on the cancellation date. There are no cancellation fees and no minimum commitment.

9. Refund Policy

See our standalone Refund Policy for full details. In summary:

  • Monthly subscription: cancel anytime before the next billing cycle and you will not be charged again
  • Unused medication is non-refundable per federal and state pharmacy regulations
  • Initial consultation fee non-refundable once the consultation has been completed
  • Lab kit fee non-refundable once the kit has been shipped
  • Billing disputes: contact support@calibr.health within 30 days of the disputed charge
  • Refund processing: 5–10 business days to original payment method

10. Patient Responsibilities

10.1 Provide accurate and complete information

You must provide truthful, accurate, and complete information in all health intake questionnaires and clinical communications. Providing false or misleading medical information — including omitting relevant medical history, current medications, or contraindications — could result in serious harm to your health. Deliberately providing false information to obtain a prescription constitutes fraud and may result in immediate termination and referral to authorities.

10.2 Keep your information current

Promptly update your health profile if your medical history, medications, contact information, or other relevant circumstances change.

10.3 Follow treatment instructions

  • Administer medications exactly as prescribed by your clinician — do not adjust doses without clinical guidance
  • Follow all instructions provided with your medication and by your clinician
  • Store medications according to the storage instructions provided
  • Attend ongoing monitoring labs (6 weeks, 3 months, then quarterly)
  • Report any side effects or adverse reactions promptly through your patient portal

10.4 Maintain a primary care relationship

We strongly encourage you to maintain a relationship with a primary care provider who is aware of your treatment. Calibr Health and our affiliated clinicians are not a substitute for comprehensive primary care.

11. Privacy

Your use of the Services is also governed by our Privacy Policy, which describes how we collect, use, share, and protect your personal information and Protected Health Information.

12. Intellectual Property

All content on the Calibr Health website and platform — including text, graphics, logos, images, design, and software — is the property of Calibr Health LLC or its licensors and is protected by U.S. and international intellectual property laws. You may not copy, modify, distribute, sell, or lease any part of the Services without our express written permission.

13. Limitation of Liability

To the maximum extent permitted by law, Calibr Health LLC, its officers, directors, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or in connection with your use of the Services. Our total cumulative liability for any claim arising out of these Terms or the Services shall not exceed the total amount you paid us in the twelve months preceding the claim.

14. Indemnification

You agree to indemnify and hold harmless Calibr Health LLC, its officers, directors, employees, and agents from and against any claims, damages, losses, liabilities, and expenses arising out of (a) your use of the Services, (b) your violation of these Terms, (c) your violation of any law or the rights of any third party, or (d) any false or misleading information you provide.

15. Governing Law

These Terms are governed by the laws of the State of South Carolina, without regard to its conflict-of-laws principles. Any disputes shall be resolved as described in Section 16.

16. Dispute Resolution & Arbitration

Please read this section carefully. By agreeing to these Terms, you agree that any dispute arising out of or relating to your use of the Services shall be resolved by binding individual arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules. You waive the right to a jury trial and to participate in a class action. You may opt out of this arbitration agreement by sending written notice to support@calibr.health within 30 days of first accepting these Terms.

Notwithstanding the foregoing, either party may bring a claim in small-claims court for disputes within the court's jurisdiction.

17. Modifications to Terms

We may update these Terms from time to time. When we make material changes, we will update the "Last Updated" date and notify account holders by email. Continued use of the Services after changes constitutes acceptance.

Each pharmacy is independently state-licensed, USP-797/795/800 compliant where applicable, and dispenses medication based on a prescription issued by a licensed clinician. Patients are matched to a partner pharmacy based on state of residence and prescribed medication. See our pharmacy partners page for more.

19. State Availability

Calibr Health currently offers services in all 50 states plus Washington D.C. Some services may not be available in all 50 states or Washington D.C. Subject to change. Service availability is determined by state telemedicine and pharmacy regulations.
